APPOINTMENTS
All patients should plan to bring their valid insurance card with them for their appointment. We may not be able to bill your insurance without the information contained on your insurance card. This may also delay your appointment or cause it to be rescheduled, depending on your insurance company’s policy.
If you are a new patient, please plan on arriving 15 minutes prior to your scheduled appointment time. This will ensure that all of the necessary paperwork is completed prior to your scheduled appointment time.
24 HOUR CANCELLATION POLICY & NO-SHOWS
If for any reason you are unable to keep your appointment, please contact our office 24 hours before your scheduled appointment if possible. By extending this courtesy, we will be able to help ensure patient visits are maintained in a timely fashion. Miller Eye Center reserves the right to charge a $10-$30 fee for any patient who does not show for their appointment and has not notified us of the need to cancel or reschedule 24 hours before their scheduled appointment (fees are based on appointment type). This policy extends to all appointments and procedures at Miller Eye Center. Any patient who does not notify us that they will be unable to arrive for their scheduled surgery may be assessed a $50 fee.
What Is a Referral?
A referral is a written authorization by a participating primary care physician approving you to receive specialized diagnostic testing or treatment from a participating provider or, under certain circumstances and with advance approval from your HMO, from a nonparticipating provider when the services cannot be provided by any of the doctors within the provider panel.
REFERRALS
